You should bring the following to the Circuit Clerk's office to apply for a U.S. Passport:  (applications are available in our office, and online at www.travel.state.gov

     1.  Passport application (DSP-11) with items 1 thru 20 completed;

     2.  Certified birth certificate or original naturalization certificate;

     3.  Two (2) identical passport photographs (from the same negative);

     4.  Driver's License;

     5.  Check or money order payable to U.S. Dept. of State for the fees noted below;

      6.  Separate execution fee payable to the Circuit Clerk (cash, check or money order).

Please do not sign the application until you are in front of an acceptance agent in the Clerk's office. 

A child 14 years and older must appear in person to sign the application.  One parent with a drivers' license needs to also be present for a child between the age of 14 and 18. 

Children under the age of 14 do not have to be present, but both parents must appear to sign the application.

The processing time by the Passport Agency is approximately 6 weeks.  Your birth certificate or naturalization certificate will be returned with your passport to the address you list on the application.  If six weeks is not soon enough to receive your passport, you must apply for expedited processing.

Expedited passport applications are processed within 2 weeks.  Requests for expedited processing require additional fees as noted below.  We recommend payment by money order or cashiers check for expedited applications.  We also recommend you bring two (2) express mail postage-paid envelopes, to speed the process.

Fees (effective 8/19/02) 

Execution Fee (payable to Circuit Clerk)                          $30

Application Fee - age 16 and up (payable to US Dept.      $55     of State)

Application Fee - under age 16 (payable to US Dept.        $40    of State)

Expedited Application Fee (Age 16 and up)                       $115

Expedited Application Fee (Under age 16)                        $100
